To submit a loan application to Crown MAC, please print and complete the following documents:

Membership Agreement
Loan Application

Personal Financial Statement
Credit Authorization Consent Form

In addition to the above, the following information will also be required to complete the loan application process.  Give us a call before you start, and we can walk you through the information gathering process.

​

• Financial statements including balance sheet and income statement

  • Three years of historical information for existing locations

  • Two years of projected information for new stores

• Complete copy of lease/sub-lease for locations being used as collateral

• Sales/purchase agreement contract, if applicable

• Legible copy of current driver's license

• Current Property & Casualty Insurance Certificate

             

             Draw/Advance or Pay-down on Working Capital/Line of Credit
